SUPPORTING YOUR DISCHARGE PROCESS

Ohana Wound Care helps make the discharge process smoother and more efficient for hospitals. We see patients within 24 to 48 hours after discharge and provide advanced wound care in the patient's home. We can work with any home health or hospice agency of your choice, or we can manage wound care independently for patients who are not yet under an agency.

INSURANCE COVERAGE

Our services are covered under Medicare Part B for eligible patients. We also accept other insurance plans on a case-by-case basis. Every patient receives a no-cost wound evaluation to determine medical need and insurance eligibility. Our team takes care of benefit verification, documentation, and billing so your discharge staff can focus on getting patients home safely.

WOUND CARE SUPPLIES

As part of our service, Ohana Wound Care provides a 30-day supply of advanced wound dressings for each qualifying wound. Supplies are delivered within 48 hours of the evaluation and restocked as needed until the wound is healed. Most standard supplies are included. Please note that certain specialty products, such as collagen and alginate, may not be covered and would need to be ordered separately.

OUR APPROACH TO CARE

We focus on healing, infection control, and helping patients recover safely at home. Our services include debridement, wound cultures, vascular assessments, cellular tissue products when appropriate, and Vitastem, an over-the-counter topical antibiotic. We coordinate closely with home health/hospice teams or family members to ensure care continues without interruption.

VISIT FREQUENCY

An Ohana Wound Care clinician will see your hospice patient within 24 to 48 hours of receiving the referral. After the initial evaluation, we provide weekly wound care visits while your team remains in charge of the patient's overall plan of care.

INSURANCE COVERAGE

Wound care services are covered under Medicare Part B for eligible patients. We also accept other insurance plans on a case-by-case basis. Eligibility is confirmed during the evaluation. All patients receive a no-cost evaluation to determine medical need and coverage. Our team handles benefit checks, documentation, and billing so your staff can focus on patient care.

ADVANCE WOUND CARE SUPPLIES

Each qualifying wound receives a 30-day supply of advanced dressings. Supplies are delivered within 48 hours of the evaluation and reordered as needed until the wound is resolved. Dressings are used by both Ohana clinicians and your hospice staff to ensure consistent care.

OUR APPROACH: COMFORT OVER CURE

We focus on comfort, not just healing. Our goal is to reduce pain, manage drainage and odor, prevent infection, and maintain skin integrity to preserve dignity and quality of life. When clinically appropriate, we provide conservative debridement, wound cultures, and vascular assessments to help guide care. We may also use cellular tissue products for stable patients when appropriate, and Vitastem, an over-the-counter topical antibiotic, to support comfort and help prevent wound-related complications. The hospice agency remains the main driver of care.

CONSISTENT ON-SITE WOUND CARE

Ohana Wound Care delivers advanced wound care directly to your facility. We treat residents once per week on a designated rounding day, allowing for consistent scheduling, reduced interruptions, and better care planning for your team. Our goal is to help improve healing outcomes while easing the demand on your in-house staff.

INSURANCE COVERAGE

Our services are covered under Medicare Part B for eligible residents. We verify eligibility during a no-cost evaluation for each referral. Ohana handles all benefit checks, documentation, and billing, so your staff does not have to.

SUPPLIES INCLUDED

Each qualifying wound receives a 30-day supply of advanced dressings, delivered within 48 hours of the evaluation and restocked as needed. Supplies are billed through Medicare Part B when applicable. Facilities have the flexibility to use their own formulary or choose to utilize Ohana's supply program to reduce costs and ensure consistency in wound care.
